Como Park Dental Associates is pleased to assist you with dental insurance filing. However, your insurance contract is between you, your employer, and the insurance company. Please be aware of deductibles, waiting periods, annual maximums, exclusions, and other limitations of your policy prior to proceeding with treatment.

As a courtesy to you, Como Park Dental Associates will file your insurance and in some instances accept assignment of benefits after you have signed the proper authorization form. Otherwise, payment will be collected at the time of service and you will be reimbursed by your insurance company.

Just as a reminder to what was said earlier not all services are covered benefits in all contracts, so please become familiar with your allowances. You are ultimately responsible for all fees for services rendered.
